
School Dental Check-Up
California law requires that all children have a dental check-up by May 31 of their first year in public school. A licensed California dentist must complete and sign the Oral Health Assessment form.

If your child had a dental check-up in the last 12 months, ask your dentist to fill out the form.

If you can’t get a check-up for your child, you’ll need to complete a Waiver Form.

Return the completed form to the school!
This dental assessment helps identify any problems early and also supports the state’s oral health programs. Good oral health is important for kids to feel confident, stay healthy, and succeed in school and life.
